00The Practice Transformation Associate position reports to the Director of Quality and serves as a key team member for achieving goals of the quality department.
Primary responsibilities include coaching care teams, reporting and analyzing clinical and operational data, and supporting quality improvement initiatives.
This work will help Esperanza maintain its national clinical quality leader status and support innovation across the organization.

Responsibilities:
Approach challenges that Esperanza's clinical & operations teams encounter with a positive outlook and solutions-focused approach to improvement and organizational learning.
We are looking for someone who is curious about complex challenges, capable of responding to feedback while advocating for our mission, and comfortable facilitating (as opposed to dictating) the quality improvement journey.
Independently schedule, facilitate, and document results of quality coaching meetings with medical and behavioral health care teams.
This work will include traveling between Esperanza's clinic locations (or collaborating virtually) to review quality measure performance, support efficient use of the electronic medical record (EMR), and enable best practices through Plan, Do, Study, Act cycles (PDSA) cycles.
Assist the department with quality reporting and analysis, which may include:
(a.
) Monthly updates of Esperanza's internal quality dashboard (b.
) Annual reporting for the Unified Data System (UDS) to satisfy health center compliance with the Health Resources and Services Administration (HRSA) (c.
) Ad-hoc requests such as lists of patients for targeted population health outreach, comparisons of quality outcomes across populations to assess health equity, or data pertaining to current or prospective grant-funded opportunities.
Proactively engage with stakeholders at weekly quality committee meetings and interdepartmental collaborations to formulate improvement ideas, generate buy-in, ensure integration of projects, and create processes and workflows that support healthcare's Quadruple Aim - lower costs, improved patient experience, better population health outcomes, and improved work experience in primary care.
Contribute key learnings to the ongoing refinement of Esperanza's Quality Improvement Plan.
The QI Plan is updated annually to ensure roles and responsibilities, policies and procedures, improvement methodologies, measure sets/definitions, and bonus incentive structures are aligned with evolving organizational goals.

PM21 Passion for people, with experience and/or a desire to perform primary care team-based quality improvement coaching.
Bachelor's and/or Master's degree in health discipline (e.
g.
MPH, MHA) with one or more years' experience in healthcare required; may substitute additional experience for educational attainment.
Data and measurement tasks must energize you, as well translating data into actionable steps to others with varying degrees of data experience and/or time.
Keen analytical ability to identify organizational problems and develop solutions.
Ability to support the implementation of large-scale projects that span the entire organization over an extended period.
Familiarity with Microsoft Office suite required, with particular focus on Excel skills; experience with PowerBI or similar data visualization tool preferred, but not required.
Familiarity with electronic medical record (EMR) software preferred, but not required.
Familiarity with quality improvement tools (e.
g.
, PDSAs, project charters, etc.
) preferred, but not required.
Knowledge of telehealth best practices & strategy preferred, but not required.
Self-driven and comfortable working remotely with minimal supervision during the COVID19 pandemic.
Must be able to report on site when required.
